FreeBSD-EN-26:22.usb Errata Notice
The FreeBSD Project
Topic: umass(4) devices may fail to attach
Category: core
Module: usb
Announced: 2026-09-29
Affects: FreeBSD 14.5
Corrected: 2026-09-04 17:56:23 UTC (stable/14, 14.5-STABLE)
2026-09-29 16:09:02 UTC (releng/14.5, 14.5-RELEASE-p1)
For general information regarding FreeBSD Errata Notices and Security
Advisories, including descriptions of the fields above, security
branches, and the following sections, please visit
.
I. Background
The USB mass storage driver, umass(4), provides support for various models
of thumb drives, hard drives, and other media devices that may be attached
over USB through an xhci(4) controller.
II. Problem Description
The umass(4) driver relies on the ability to reset the USB data toggle
during normal operation. A change to xhci(4) intended to keep the data
toggle state synchronized with the connected device inadvertently prevented
device drivers from resetting it.
III. Impact
Some umass(4) devices may fail to attach as a result of this change. One
such failure may be surfaced via a bad CAM status, such as:
(da8:umass-sim0:0:0:0): got CAM status 0x444
(da8:umass-sim0:0:0:0): fatal error, failed to attach to device
IV. Workaround
No workaround is available.
V. Solution
Upgrade your system to a supported FreeBSD stable or release / security
branch (releng) dated after the correction date, and reboot the system.
Perform one of the following:
1) To update your system installed from binary distribution sets:
Systems running a RELEASE version of FreeBSD on the amd64 or arm64 platforms
which were not installed using base system packages can be updated via the
freebsd-update(8) utility:
# freebsd-update fetch
# freebsd-update install
# shutdown -r now
2) To update your system via a source code patch:
The following patches have been verified to apply to the applicable
FreeBSD release branches.
a) Download the relevant patch from the location below, and verify the
detached PGP signature using your PGP utility.
# fetch https://security.FreeBSD.org/patches/EN-26:22/usb.patch
# fetch https://security.FreeBSD.org/patches/EN-26:22/usb.patch.asc
# gpg --verify usb.patch.asc
b) Apply the patch. Execute the following commands as root:
# cd /usr/src
# patch -E -p0 < /path/to/patch
c) Recompile your kernel as described in
and reboot the
system.
